Remove dependency on couch_db.hrl

Description

Currently macros defined in couch_db.hrl are used. Copy those over to vtree.hrl, so you don't need to include couch_db.hrl any longer.

Wasn't a good idea. In cases where you need to include both, the vtree.hrl and the couch_db.hrl the macros will clash.
